Weil syndrome with vaginal bleeding: a case report

Leptospirosis is a common zoonosis in Brazil, with an incidence of approximately 1.9 cases per 100,000 inhabitants. It is caused by the Leptospira bacteria, transmitted by contact with contaminated water, soil or animal tissue. Clinical manifestations vary depending on the early or late phase, ranging from nonspecific symptoms, such as fever, headache, myalgia and vomiting, to severe organic dysfunctions, such as Weil Syndrome (jaundice, kidney damage and hemorrhages). This paper describes a 51-year-old woman with fever, jaundice, pain in the lower limbs and moderate vaginal bleeding. She was diagnosed with Weil syndrome with vaginal bleeding based on clinical and laboratory history and positive serology for leptospirosis. Antibiotic therapy was started, progressing with improvement and consequent hospital discharge. As it is an underreported disease, it is necessary to identify symptoms and possible atypical manifestations in order to facilitate early diagnosis, in addition to developing preventive strategies in an attempt to minimize costs and reduce complications.
