Xanthogranulomatous cholecystitis simulating gallbladder cancer: case report

Xanthogranulomatous cholecystitis is a benign and rare inflammatory gallbladder disease, characterized by a destructive inflammatory process and irregular thickening of the gallbladder wall, a high chance of infiltration of adjacent organs and complications. Due to its chronic and invasive nature, it can simulate gallbladder cancer. The definitive diagnosis is given by the postoperative histopathological evaluation. The present study reports the case of a patient with pain in the upper right abdominal quadrant, vomiting and weight loss in 4 months, with an initial hypothesis of gallbladder cancer. However, despite preoperative and intraoperative evaluations compatible with the initial suspicion, the postoperative histopathological diagnosis was chronic xanthogranulomatous cholecystitis. The purpose of the study is to discuss concepts about the subject and the approaches adopted in the case, highlighting the importance of the availability of intraoperative biopsy to prevent more aggressive surgeries and unnecessary increases in morbidity and mortality to the patient.
